I, ________ (full name listed above) hereby authorize Hope Credit Union hereafter referred to as "HOPE," to publish photographs, video and/or my personal story/testimonial and give HOPE permission to use my name, likeness, and personal story /testimonial in HOPE's print, online and video-based marketing materials, as well as other HOPE publications/presentations. I hereby release and hold harmless HOPE from any reasonable expectation of privacy or confidentiality associated with the images and story specified above. I further acknowledge that my participation is voluntary and that I will not receive financial compensation of any type associated with the taking or publication of these photographs, video, personal story/testimonial or participation in company marketing materials or other HOPE publications. I acknowledge and agree that publication of said photos, videos, personal story and testimonial confers no rights of ownership or royalties whatsoever. I hereby release HOPE, its contractors, its employees, and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me, my heirs, representatives, executors, administrators, or any third party in connection with my participation.
